Dungeons & Dragons 2024 (D&D) Item: Lesser Behemoth Mutagen (Psion)

POTION RARE ALCHEMICAL MUTAGEN

A vial of a thick purple liquid that seems to glow softly.   When consumed by a Humanoid it polymorphs them into a Lesser Psion Behemoth for 1 hour. The character must make a Wisdom Saving Throw DC15, on failure they lose control and will attack the closest creature that is not Unconscious. If the creature is rendered Unconscious the character will make another Wisdom Saving Throw DC15, on failure it attacks the next closest creature that is not Unconscious.   When the Behemoth is reduced to 0 hit points it reverts back into the character. The character is also at 0 hit points and starts making Death Saving Throws.
